Why Some Leukaemia Cases Are Harder to Treat
Most patients with Acute Lymphoblastic Leukaemia begin treatment on a standard chemotherapy protocol and for many, that protocol works. But leukaemia does not always follow the expected path. Some patients relapse. Some stop responding to the drugs that once worked. For those families, the question of what is still possible becomes the only one that matters and until recently, the answers available in India were limited.
The Treatment That Is Changing the Odds
Over the past two years, a small number of Indian centres have begun offering CAR-T cell therapy for leukaemia, a treatment that was largely unavailable in the country before that.
The way it works is straightforward to explain, even if the science behind it is not. The patient's own immune cells are collected, reprogrammed to find and attack the leukaemia cells that chemotherapy could not reach and returned to the body. The immune system then does what it was never able to do before. In patients with relapsed B-cell leukaemia, this approach has produced remission in 80 to 90 percent of cases, a number that means everything to families who had been told there was nothing left to try.
India now has its own approved product for this therapy. India now has an approved indigenous CD19 CAR T-cell therapy , making this advanced treatment more accessible for eligible patients within the country.

What the Treatment Journey Looks Like
The process spans several weeks. Cells are collected in a short-stay procedure that most patients find far less daunting than they expected. Those cells are then engineered over two to four weeks in a lab. When they are ready, the patient goes through a short course of conditioning treatment before the infusion itself, which takes only a few hours. After that, the medical team monitors the patient closely for about two weeks as the immune response builds.
What happens after discharge matters as much as what happens during treatment. Check-ins, blood tests, and bone marrow assessments typically continue for years, this is not a therapy where a family is handed a discharge summary and left to manage the rest on their own.
Understanding the Side Effects
Centres delivering this therapy are upfront with families about what to expect. The main side effects are well documented and are watched for closely from the moment of infusion. Most patients manage them without serious complications when the treating team has handled these responses before, which is precisely why the therapy is currently offered at only a small number of specialised centres rather than every hospital.
What Families Should Look for in a Treatment Centre
Because CAR-T therapy is still available at only a handful of hospitals in India, the details of where it is delivered matter almost as much as the therapy itself. A few things consistently come up when families and doctors discuss what makes a difference:

Purpose-built infrastructure. Sterile environments and specialised isolation rooms are not something a general hospital ward can simply add on, they need to be designed for this kind of work from the outset.

Long-term follow-up. Because outcomes are monitored for years after treatment, centres with a structured follow-up programme tend to catch complications earlier.
